  乳癌手術的新進展

  “請醫師幫我用微創手術”、“我想用內視鏡拿掉我胸部腫瘤”是目前網路上最熱門的話題。其實，這些都是整型式乳癌切除手術(oncoplastic surgery) -- 採用整型外科的技術來增加切除安全距離且改善乳房術後的外觀的作法，手術包括乳癌切除(onco-)及乳房整型(plastic)兩部份，簡介如下。

  微創內視鏡手術用於“乳房全部切除”：乳房全部切除是對於多發性病灶或大型腫瘤的必要手術，然而手術後會留下長長的疤痕。若皮膚或乳頭必須沒有癌細胞侵犯，運用內視鏡或機器手臂，保留乳房皮膚及乳頭，可以3-4公分的腋下傷口切除乳房並植入義乳，術後乳房本身完全無傷口。手術健保不給付需自費。值得注意的是，乳房全部切除是一個較大的手術，同時為了取出整個乳房的“微創”傷口並非微小，重建後的皮膚通常失去觸感，並且有義乳植入需要後續照顧。

  整型式乳房保留手術：在數十年前開始，美國及歐洲醫師分別展開大型隨機前瞻性實驗，比較乳房全部切除和乳房部份切除的療效。後來同時於2002年在新英格蘭醫學雜誌發表了20年的研究報告證實兩者存活率一樣。因此在單一病灶及腫瘤較小的病患，乳房保留手術成為乳癌手術的標準治療。

  整型式乳房保留手術用局部乳腺移動彌補所切除的乳房缺損，依腫瘤位置及分布，有以下幾種作法: 平行四邊形切法(針對局部生長型腫瘤)、蝙蝠翼切法(針對局部生長型腫瘤位於乳頭下方)、甜甜圈切法(針對乳管系統分布型腫瘤位於乳房外上側) 、縮乳切法(針對乳管系統分布型腫瘤位於乳房下側) 、中央切法(針對局部生長型腫瘤需切除乳頭乳暈)。不過，依個人情況不同，手術後可能有患側乳房較小、疤痕增生及兩側乳頭高低不對稱等缺點，而且術後必須加做放射治療，有健保給付。

  展望：台灣地區隨著診斷工具的進步及全民篩檢，早期乳癌(特別是原位癌)，將來會成為接受治療的主要病人群。因此適當地選擇病患給予成功的整型式乳癌手術，不但可以治癒乳癌，也可以保有美觀的乳房；對於患者的身體及心理都有莫大的幫助。
