服務內容---

★醫師訪視

　經團隊評估個案需求，安排醫師居家訪視服務，皆依據健保規範執行相關業務，例如：協助開立居家醫療醫囑等。

醫師居家訪視時的照片

★居家護理師訪視

  • 依個案需求並經醫師評估後，進行居家訪視，提供技術性等護理服務。
  • 提供一般照護、特殊照護及自我照護指導。
  • 指導主要照顧者照護照護知識及技巧。
  • 進行個案健康管理，依健康狀態連結醫療及長期照顧服務資源。

★營養師訪視

　經居家照護團隊評估後，依個案需求與營養師共同家訪，協助訂定飲食計畫，如:糖尿病、高血壓等飲食諮詢及指導等。
