«住院安寧緩和療護流程»

本院住院病人及急診病人：經由原團隊主治醫師會診安寧共照，由安寧專科醫師評估後收住。
本院門診病人：經掛號安寧專科醫師，由安寧專科醫師評估後收住。
仁心安寧居家病人：經由仁心居家護理師聯繫，經由安寧專科醫師評估後收住。
外院病人：由家屬攜帶病歷摘要或以傳真方式，經由安寧專科醫師評估後安排收住。
